Getting Started Questions

What is this book about?

This is an evidence-based guidebook about Attention-Deficit/Hyperactivity Disorder, written for two readers at once: the person who has ADHD and the people who love and support them — parents, partners, siblings, and close friends. It combines the science of ADHD (what it is, how the brain works, how diagnosis and treatment actually operate) with practical strategies for daily life, school, work, and relationships. The organizing idea is that a diagnosis lands on a whole family, not just one person, so every major topic addresses both sides. See the course description for the full scope.

Who is this book for?

Two readers, by design. First: adults and teens who have been diagnosed with ADHD or suspect they have it — with particular attention to adults diagnosed late and to women whose ADHD went unrecognized for years. Second: the people around them — parents (including parents of grown children), spouses and partners, siblings, close friends, and teachers or mentors without specialized training. No medical background is assumed; every clinical term is explained in plain language when it first appears. The book works best when both readers read it and compare notes.

Do I need any background knowledge to read this book?

No. There are no prerequisites. The book is written at a 9th-10th grade reading level, and every concept is built from the ground up: Chapter 1 defines attention itself before it defines ADHD, and Chapter 2 explains the brain science without assuming you have ever taken a biology class.

In what order should I read the chapters?

Front to back works best the first time, because later chapters deliberately build on vocabulary from earlier ones — the "regulation lens" from Chapter 1 and the executive-function toolkit from Chapter 2 get used everywhere afterward. But the book is also built as a reference: jump to Chapter 5 before an evaluation, Chapter 7 before a prescriber visit, or Chapter 12 before a school meeting. Each chapter opens by naming which earlier chapters it builds on.

What are the MicroSims and how do I use them?

How should a family use this book together?

The book is built for shared reading: the person with ADHD and a family member read the same chapter and compare what rang true. Many chapters include a "For both readers" exercise designed to start that exact conversation — trading examples of attention steering in Chapter 1, mapping your household's executive-function trouble spots in Chapter 2, negotiating the help menu in Chapter 14. Reading together is not just preparation for helping; family psychoeducation is itself an evidence-backed intervention, as Chapter 8 explains. The two capstone documents in Chapter 15 — one written by each reader, then exchanged — are where the shared reading lands.

Can this book diagnose me or someone I love?

No, and it says so plainly. The book explains what ADHD is, what an evaluation involves, and what a diagnosis means, so you can walk into the process informed — but only a qualified professional can diagnose, through the kind of thorough evaluation Chapter 5 describes. If reading the early chapters produces a strong feeling of recognition, the book's advice is consistent: treat that recognition as a valid reason to seek a proper evaluation, never as the evaluation itself.

Does this book give medical or medication advice?

No. Chapter 7 explains how ADHD medications work, what titration is, and what questions to bring to appointments — but it contains no dosing advice and recommends no specific medication for any specific person. Every treatment decision in this book is framed the same way: informed by evidence, made with a qualified clinician.

How much time does the book take?

There are fifteen chapters totaling roughly 65,000 words — about the length of a short nonfiction book. A chapter takes most readers 20-35 minutes plus time with its MicroSims. Read one chapter a week with a shared conversation afterward and the whole book is a season; read it straight through and it is a long weekend. The ADHD-friendly advice from its own toolkit applies: a chapter a week on a standing calendar slot beats an ambitious plan that collapses by Thursday.

What makes this book different from other ADHD books?

Three commitments. First, the dual audience: every major topic addresses the person with ADHD and their caregivers together, so both sides read the same book and meet in the middle. Second, special emphasis on the people the system misses — late-diagnosed adults, and girls and women whose masking hid the condition for decades (Chapter 4). Third, a strengths-honest stance: the book takes ADHD seriously as a disability where it is one and seriously as a difference with real strengths, without romanticizing either. It also treats supporting an adult without parenting them as a first-class subject (Chapter 14), and gives caregivers their own care chapter (Chapter 15).

Where do I start if my family member was just diagnosed?

Read Chapter 1 first — it retires the myths (laziness, willpower, intelligence) that quietly damage families, and gives you the shared vocabulary everything else uses. Then read the section of Chapter 5 called "What a Diagnosis Means — and Doesn't." If the person diagnosed is an adult, read Chapter 14 on supporting without parenting before you change anything about how you help. Core Concept Questions

How is attention actually supposed to work?

Attention is the brain's ability to select some information for processing and set the rest aside — a spotlight with an operator. It is really several skills working together: selecting a target, sustaining on it, resisting capture by louder or newer input, shifting deliberately when the task changes, and dividing when you must. ADHD is a problem with the operator, not the light: each of those steps costs more, especially for uninteresting targets. What does the prefrontal cortex do?

The front third of the brain, directly behind the forehead — the executive suite. It holds goals in mind, plans steps, suppresses derailing actions, weighs consequences, and adjusts when circumstances change. Two of its properties explain most of this book: it matures last (into the mid-20s, and later still in ADHD), and it is expensive to run — depleted by fatigue, stress, hunger, and poor sleep, which is why anyone's self-control is worse at 11 pm and why afternoon collapses after composed mornings are chemistry, not character. What is internal restlessness?

The hyperactive motor felt from inside with the outside stilled: a crawling, revving, can't-settle sensation — a mind that channel-surfs itself, a body that finds stillness genuinely uncomfortable even while sitting perfectly still. It is the form hyperactivity takes as people age and social pressure clamps the visible version, and it is among the most commonly reported adult symptoms — and the least visible to families. An adult saying "I can't relax, my brain won't shut up, vacations stress me" is reporting a core symptom, not ingratitude. See Chapter 3.

What does ADHD look like in childhood?

Symptoms at their most visible: motor hyperactivity at its lifetime peak, impulsivity without social brakes, inattention colliding with school's first sit-still demands — which is why most diagnoses happen between six and nine, usually flagged by a teacher. The stage's blind spot: referral runs on disruption, so the child who bothers others gets evaluated while the quiet daydreamer reading two grade levels ahead never trips the alarm. And because parents and teachers do the executive functioning at this age, a well-scaffolded child can look nearly fine — support that quietly hides a skills gap the later transitions will expose. How does ADHD change in the teenage years?

Three pressures converge. The visible motor goes underground — parents watch hyperactivity "fade" while inattention quietly takes over the grade book. Demands jump — seven teachers, long-horizon projects, and a planner-based life arrive exactly when adult supervision is expected to back off. And stakes get real: driving, substances, a permanent record. It is also when the gap versus peers looks widest, because classmates' self-management is accelerating while the maturation lag holds. The move that works is scaffolding with the teen rather than surveillance around them — systems they'll own pieces of, because the goal is transfer before the college cliff. See Chapter 4.

What are the most damaging myths about ADHD?

The ones that convert symptoms into accusations. The laziness myth ("they just don't want to work") ignores the tell that separates ADHD from laziness: the suffering — lazy people are untroubled by not working; people with ADHD sit in escalating misery in front of the undone task. The willpower myth ("just try harder") prescribes the one strategy guaranteed to have already failed for years before anyone reaches an evaluation. How does stigma actually hurt people with ADHD?

Through a feedback loop, not single insults. Public stigma (the jokes, the "excuse" talk) makes people hide symptoms and delay evaluation; delayed evaluation means years of unexplained struggle; unexplained struggle gets explained anyway — by the myths; the person internalizes those verdicts as self-stigma, which many late-diagnosed adults say hurt more than the symptoms; and the worsened visible struggle refreshes the public stigma. Families sit at the loop's two best cutting points: accurate information, and responding to symptoms with the regulation lens instead of character verdicts — because for one person's daily life, family is the loudest culture there is. What happens to a family when one member has ADHD?

The system reorganizes around it, in patterns so consistent therapists can sketch them blind: one member absorbs the executive load and converts into a logistics department (the compensator); someone becomes the household's compliance officer (the auditor); the sibling without ADHD learns to need less and shine quietly; and resentments compound on both sides' ledgers. None of these roles is chosen — all are drift — and naming the system is itself the intervention, because a family that can say "we've drifted into compensator-and-auditor" has stopped arguing about who's lazy and who's a nag and started redesigning a system. What is ADHD?

ADHD — Attention-Deficit/Hyperactivity Disorder — is a neurodevelopmental disorder defined by a persistent pattern of inattention and/or hyperactivity-impulsivity that is stronger than expected for a person's age, shows up across multiple settings, and genuinely interferes with life. "Neurodevelopmental" means it is brain-based, present from childhood, and lifelong — not caused by parenting, sugar, screens, or character. The name is admittedly a poor fit: attention isn't absent in ADHD, it steers unevenly. Chapter 1 builds this definition piece by piece.

What does "ADHD is a regulation difference" mean?

It is the single most useful sentence in the book: the core of ADHD is difficulty regulating — attention, action, impulses, emotions, motivation — rather than a shortage of any of those things. The capability is present; the management of it is inconsistent. This lens re-explains the observations that confuse families most: the child who plays chess for an hour but can't do five minutes of homework, the adult who hyperfocuses on a project all night but can't start the taxes. Through a deficit lens, those look like choices. Through a regulation lens, they are steering patterns — and steering patterns can be engineered around, which is what the whole second half of the book does. See Chapter 1.

What is executive function?

Executive function is the umbrella name for the mental skills that manage other skills: holding information in mind while using it (working memory), stopping an action already loading (response inhibition), switching between tasks (cognitive flexibility), breaking goals into steps (planning), converting intention into action (task initiation), and tracking how you're doing (self-monitoring). Knowing how to do homework is a skill; getting yourself to start it is an executive function. ADHD is, in practical terms, executive function running inconsistently — which is why "but she knows how to do this" misses the point. Chapter 2 gives each component a plain-language name and a worked example.

Example: A teen who aces the history test (knowledge) but leaves the finished permission slip on the counter (prospective memory) is showing exactly the split executive function predicts.

What is working memory and why does it matter?

Working memory is the mental desk — the small workspace where you hold information while using it: a phone number while dialing, the first half of an instruction while hearing the second. It is small for everyone and smaller or leakier in ADHD. When it drops something, the information was never filed at all, which is why "retracing your steps" fails and why repeating an instruction louder doesn't help. The fix is never "listen harder" — it is writing things down at the moment they arrive, because paper has no capacity limit. See Chapter 2 and the externalizing systems in Chapter 9.

Example: "Print the deck, use Dana's updated numbers, and book the small room" is three items on a desk that holds about three items — one of them usually slides off.

What is prospective memory?

Prospective memory is memory for intentions — remembering to do things at the moment they're due: take the medication at 8, bring the form Thursday, pick up the prescription after work. Its brutal feature is that nothing prompts it; the remembering has to fire by itself at the right instant. In ADHD it is among the most reliably impaired functions, and it is the single greatest generator of relationship damage, because from the outside a failed prospective-memory alarm is indistinguishable from not caring. The fix is refusing to let intentions live only in a brain: alarms set at the moment of promising, shared lists, items placed on the driving route. What does dopamine have to do with ADHD?

Dopamine is the neurotransmitter most tied to motivation and reward anticipation — the brain's tagging system for "this matters, move toward it." In ADHD, dopamine signaling in the brain's reward circuits appears to run differently: weaker tagging for distant or mild rewards, stronger relative pull from immediate, novel, or intense ones. That is the engine under most everyday ADHD patterns — the deadline sprint, the abandoned hobby, the video-game hyperfocus. It is also why most ADHD medications work on dopamine and its partner norepinephrine. What it is not: a simple "dopamine deficiency" fixable with supplements — Chapter 1 takes that sales pitch apart, and Chapter 2 gives the honest version.

What is delay aversion, and why do deadlines change everything?

Everyone values a reward less as it moves away in time; in ADHD that discounting runs much steeper, so a reward three weeks out is motivationally almost invisible. This explains the pattern every family eventually notices: the assignment due tomorrow gets done tonight in ninety focused minutes, while the identical assignment due in three weeks sits untouched for twenty days — then gets done in the same ninety minutes the night before. Same student, same ability; the only variable was when the payoff entered motivational range. Deadlines don't create the ability, they move the reward close enough to see. What is the interest-based nervous system?

A clinician's label (from William Dodson) for what reliably activates ADHD motivation: interest, novelty, challenge, urgency, and often competition or an audience — as opposed to the importance-based activation (priorities, obligations, consequences) that neurotypical planning runs on. It is a clinical description rather than a formal neuroscience category, but families find it immediately usable: stop asking "how important is this?" — a currency the system barely accepts — and start asking "how can this become interesting, novel, urgent, or shared?" Every practical trick in Chapter 9 — body doubling, gamified chores, artificial deadlines — is a currency exchange into one of those four.

Why is the ADHD brain described as maturing late?

Landmark brain-imaging studies that followed hundreds of children found the cortex in ADHD reaching its developmental milestones on the normal trajectory but roughly two to three years later — with the delay largest in the prefrontal regions that handle planning and self-control. That is why an 8-year-old with ADHD can seem, specifically in self-control terms, like a typical 5- or 6-year-old: in that dimension the comparison is close to literal. Three careful readings matter: it is a group average, not a measurement of your child; "lag" doesn't mean full catch-up — some differences persist into adulthood; and the lag is invisible from outside, so children get billed for hardware that isn't installed yet. What are the three presentations of ADHD?

The DSM-5 recognizes eighteen symptoms in two columns — nine inattentive, nine hyperactive-impulsive — and which column meets the diagnostic threshold determines the presentation. Predominantly inattentive: the daydreamer, the quietly drowning student, disproportionately girls and women, disproportionately missed. Predominantly hyperactive-impulsive: the classic visible picture, most common in young children and the rarest overall. Combined: both thresholds met, the most common in clinics. Crucially, presentations are snapshots that shift with age — a hyperactive child often becomes an inattentive-looking adult as the motor turns inward. What is hyperfocus?

Hyperfocus is intense, tunnel-vision absorption — hours vanishing into a game, a project, or a rabbit hole, with time, hunger, and interruptions all muted. It is the other face of the same regulation coin: the spotlight that resists steering toward dull tasks also resists steering away from gripping ones. It is genuinely double-edged — aimed well, it produces extraordinary work; aimed anywhere, it makes a person unreachable and startled by nightfall. One family-relevant fact: being yanked out of hyperfocus is jarring and often triggers a snap that reads as hostility, so households do better with a negotiated interruption protocol than with shouted demands from another room. What is time blindness?

Time blindness is a weakened felt sense of time — how long things take, how much has passed, how far away a deadline sits. Researchers describe the ADHD time horizon as collapsed to "now" and "not now": a deadline is motivationally invisible until it crosses into now. Daily life: chronic lateness that surprises the person themselves, "five more minutes" that was forty, and the sincere belief that getting ready takes eight minutes when it has taken twenty-six every day this year. The fix is making time visible and external — analog clocks, countdown timers, and alarms at the start of getting-ready rather than at departure. What is masking?

Masking is the continuous, effortful concealment of symptoms — suppressing the fidget, rehearsing sentences to avoid blurting, triple-checking everything to hide the forgetting — plus the constructive twin, compensation: self-built workarounds that deliver normal-looking results at abnormal private cost. Three facts organize everything the book does with it: masking works, which is why masked ADHD goes undiagnosed for decades; it is expensive, spending the same executive budget everything else needs, with the bill arriving at home; and it corrodes from inside — exhaustion, anxiety, and the loneliness of being valued for a performance. Why does my family member do fine some days and fall apart on others?

Because variability is the signature of the condition, not evidence against it. Symptoms genuinely change strength with the situation — interest, novelty, urgency, one-on-one attention, and immediate feedback all reduce them — and with the day: sleep, load, and stress move the executive budget. The trap families fall into is weaponizing the good day: "you did it perfectly last week, so I know you can" converts a best-ever performance into the new minimum standard. The accurate reading of a great day is "conditions lined up," and the useful move is asking which conditions so you can line them up on purpose. Chapter 3 explains both concepts.

Is ADHD really 70-80% heritable?

Yes — twin studies consistently put ADHD's heritability around 70-80%, roughly the heritability of height. Identical twins, who share all their genes, match on ADHD far more often than fraternal twins, who share half. There is no single "ADHD gene"; hundreds of common variants each nudge the odds. Two family implications: a child's diagnosis is frequently the clue that leads to a parent's own late diagnosis (the waiting-room moment in Chapter 1), and guilt about "passing it on" deserves retirement — nobody chooses their genes, and the same inheritance carries the strengths.

What actually causes ADHD — and what doesn't?

The evidence points to genes, heavily (70-80% heritability), plus modest early biological risk factors: prematurity, low birth weight, and prenatal exposure to alcohol, tobacco, or lead. The well-studied list of things that do not cause ADHD: sugar (tested repeatedly; the effect isn't there), screens and video games (they can worsen symptoms, they don't create the condition), parenting style (chaotic homes amplify struggles; calm homes contain plenty of ADHD), and vaccines (no credible evidence, ever). If someone in your family is quietly blaming themselves, this list is for them. See Chapter 1.

Why do girls and women get diagnosed so much later?

Because the referral system runs on disruption, and girls with ADHD disrupt less. They are more likely to have the inattentive presentation — the daydreamer by the window, not the boy out of his seat — and their struggle tends to turn inward as anxiety, perfectionism, and quiet shame rather than outward as behavior anyone flags. High-achieving girls add the cruelest layer: intelligence plus perfectionism builds a compensation machine that produces good grades while the private cost accumulates, so the diagnostic system's impairment requirement sees nothing wrong. The result: childhood diagnosis ratios near three boys per girl, converging toward parity in adulthood — a backlog of missed girls arriving at clinics decades late, typically via a crisis or their own child's diagnosis. Does ADHD go away with age?

Mostly no — longitudinal research shows a substantial majority of childhood cases carry impairing symptoms into adult life. What changes is the costume: visible hyperactivity usually goes inside, becoming internal restlessness, overcommitment, the jiggling knee, and the mind with forty tabs open. An adult who "sits still now" hasn't necessarily outgrown anything; the presentation shifted while the invisible half kept running. Adult prevalence runs around 2.5-4%. Chapter 3 covers the shift; Chapter 4 follows it across the lifespan.

What is emotional dysregulation, and is it part of ADHD?

Yes — emotions that arrive faster, hit harder, and take longer to release are part of ADHD's core machinery, even though they were dropped from the formal diagnostic criteria along the way. The mechanism is the same brake that fails on blurted words failing on feelings headed for expression. Two facts change how households read the storms: the reaction is real, not theatrical — nobody chooses the surge, only what happens next — and the storm passes fast, typically a squall rather than a front, which is why the person is often puzzled that everyone else is still shaken an hour later. What is rejection sensitive dysphoria (RSD)?

RSD is the community's name — coined by clinician William Dodson, and not a formal diagnosis — for a specific, brutal pattern: disproportionate, sometimes physically painful emotional collapse in response to rejection, criticism, or perceived failure, including rejection that turns out to be imagined. The "ok" text read as relationship-ending; the one item of feedback that erases the week. Its signature is speed and totality — it arrives as fact, in the body, not as a thought to evaluate. Knowing its name is genuinely therapeutic: "this is the RSD spike, it's a false alarm at least half the time, and it will pass." Are there real strengths that come with ADHD?

The book's answer is deliberately two-sided. No, ADHD is not a superpower — the impairments are real, and telling a drowning person they're secretly lucky insults the water. And yes, the strengths are real: divergent creative thinking (with actual research behind it), energy and enthusiasm that can outwork a building when pointed at loved work, crisis calm, and a resilience forged by decades of adapting and rebuilding. Some strengths are the same traits that impair, running in contexts that reward them. Naming them matters practically, not decoratively: Chapter 13 argues that designing life around your traits beats fighting them, and you can't design around strengths you haven't inventoried. What is cognitive load, and why does it explain so many evening fights?

Cognitive load is the total demand on working memory and executive systems at a given moment — every instruction held, decision pending, and distraction resisted draws on one small budget. ADHD's budget is smaller and drains faster, and load is invisible, so observers bill people for failures of an account they can't see. "Empty the dishwasher" is trivial on a fresh budget and genuinely undoable at 9 pm after a day of masking at school or work — and the snap it provokes reads as attitude when it's arithmetic. The load lens converts whole categories of family fight into engineering problems: fewer simultaneous demands, instructions on paper, and hard conversations scheduled for funded hours. Technical Detail Questions

How do I read an evaluation report?

Like a map, not a title page. Check the conclusion's specificity (a real one names the presentation); find the evidence trail (interview findings, scale scores, collateral, childhood documentation); read the differential paragraph closely — co-occurring diagnoses listed there set up everything that follows; and mine the recommendations section, which is frequently the most valuable and least read part: treatment directions, accommodation language schools and employers will accept verbatim, and referrals. Store the document somewhere retrievable — accommodation processes will ask for exactly this file for years. What does the DSM-5 actually require for an ADHD diagnosis?

Five gates, all of which must be met: at least 6 of 9 symptoms in a category (5 for people 17 and older); present at least six months at a level out of step with developmental age; several symptoms present before age 12; symptoms in two or more settings; and clear interference with functioning — plus a check that nothing else explains the picture better. The counting rules are what separate a lifelong regulation pattern from a rough semester. Chapter 1 walks the criteria; Chapter 5 shows how evaluators apply them.

What's the difference between ADD and ADHD?

ADD (Attention Deficit Disorder) was the official name from 1980 to 1987 and survives in everyday speech, but it hasn't been a diagnosis for decades. Today there is one condition — ADHD — with three presentations, and what people mean by "ADD" is now "ADHD, predominantly inattentive presentation." If a grandparent or an old school record uses ADD, nothing is wrong; the vocabulary is just from a previous edition. See Chapter 1.

What does a thorough ADHD evaluation include?

Every legitimate evaluation is built from the same components: a long clinical interview (the spine — 60-90 minutes walking current symptoms, history back to childhood, sleep, family history), standardized rating scales with population norms, self-report questionnaires, collateral information from people who see the person daily, and childhood evidence such as old report cards. Neuropsychological testing is optional — valuable for complicated pictures, not required for diagnosis — and a computer attention test is one data point, never the verdict. Who is qualified to diagnose ADHD?

In the US, three main figures: psychiatrists (medical doctors who can diagnose and prescribe — strongest for complicated pictures), psychologists (doctoral-level clinicians who conduct the most thorough testing but mostly don't prescribe), and primary care providers (who diagnose much real-world ADHD, best for straightforward childhood presentations). Some other licensed professionals can diagnose depending on the state. The license matters less than the process — which is why Chapter 5 teaches you to judge the evaluation itself.

What are stimulant medications and how do they work?

Stimulants are the first-line ADHD medications, in exactly two classes: methylphenidate (the molecule in Ritalin and Concerta) and amphetamines (the molecules in Adderall and Vyvanse). Both raise dopamine and norepinephrine signaling in the prefrontal circuits that do the steering — methylphenidate mainly by blocking the "vacuum" that recycles those messengers, amphetamines by also nudging more release.
